David G. Lamb, Ph.D., ABPP-CN
Dr. Lamb is a board certified clinical neuropsychologist who worked for over eleven years at the Center for Transitional Neurorehabilitation (CTN) of the Barrow Neurological Institute (BNI) in Phoenix, AZ. While a staff neuropsychologist at CTN, he conducted neuropsychological assessments, provided individual and group psychotherapy, administered cognitive retraining tasks, and ran an educational group for patients. He also assisted patients in creating strategies and techniques for overcoming brain injury related difficulties, both in the clinic and on-site at their actual places of employment. In July of 2003, Dr. Lamb transitioned to private practice and is offering services in the areas of neuropsychological evaluation, neurorehabilitation, and psychotherapy. In addition to his expertise as a clinical neuropsychologist, Dr. Lamb holds a Master’s Degree in Art Therapy, allowing him to take a unique approach to mental health that utilizes artistic methods to diagnose and treat people with psychological difficulties. He has been involved in facilitating a support group sponsored by the Brain Injury Alliance of Arizona (BIAAZ) for young adult survivors of TBI and their families since October of 2006.
After earning a B.S. in Psychology from Colorado State University, Dr. Lamb continued his education at Emporia State University (ESU) in Kansas. By the time he had completed his studies at ESU, he had obtained a second B.S. in Art and separate Master of Science degrees in both Art Therapy and Clinical Psychology. In 1992, Dr. Lamb was awarded his doctorate in Clinical Psychology from the University of Kentucky, where he took relevant coursework and received specialized clinical training in neuropsychology. He then completed his two-year postdoctoral fellowship in clinical neuropsychology at the BNI in Phoenix. Dr. Lamb was awarded certification in Clinical Neuropsychology by the American Board of Professional Psychology in August of 1999.
Dr. Lamb is a full professional member of the American Academy of Clinical Neuropsychology and National Academy of Neuropsychology.
Mattie Cummins, M.S.W., LMSW
Mattie Cummins is a social worker and has worked with people with brain injuries and their families for over 18 years. She has also provided direct social work services through a number of nonprofit organizations for people with varying disabilities, including spinal cord injuries, strokes, and other neurological disabilities. In addition, she has served as the executive director of the Brain Injury Alliance of Arizona, which has provided her experience in managing service programs, including Kamp Kan Do and other support programs. She provides education on people with brain injuries to professionals throughout Arizona.
Ms. Cummins has a Bachelor’s degree in Social Work from Texas A&M University at Commerce and a Master’s degree of Social Work from the University of Texas at Arlington. She is a Licensed Master Social Worker in Arizona. She currently serves as a member of the Community Action Board for the ASU School of Social Work.
Marcos Molinar, M.S., C.R.C.
In addition to his work with AzN, Mr. Molinar has established and continues to provide services through his private vocational rehabilitation counseling practice. He has maintained Certified Rehabilitation Counselor (CRC) status since 2001, primarily focusing on clinical counseling individuals with disabilities. Mr. Molinar has been a member of the International Association of Rehabilitation Professionals (IARP) since 2004 and in 2009 began working in leadership roles for IARP, including Arizona IARP Chapter President from 2013 – 2015. He also provides continuing education presentations to rehabilitation, medical, and human resource professionals at international conferences and local seminars. Mr. Molinar is a Vocational Expert for the Social Security Office of Disability Adjudication and Review, providing gainful employment testimony for social security applicants since 2013. He has been working in a Neurorehabilitation therapist role since 2004.
Mr. Molinar received a Bachelor of Arts in Psychology in 1999 and a Master of Science degree in Rehabilitation Counseling in 2002, both from the University of North Texas. He completed the Forensic Rehabilitation Counseling Certificate program at George Washington University in August 2011. He added Forensic Rehabilitation as part of his practice in 2009, specializing in cases of survivors of traumatic brain injury and cognitive disabilities.
Deborah Taylor, PT
Deborah Taylor is a state licensed Physical Therapist. She has worked in rehabilitation since 1976, when she began her career at Good Samaritan Rehab, where she worked for five years in all areas of physical therapy, but particularly with spinal cord injury patients. Mrs. Taylor then moved to the Outpatient Services Department at St. Joseph Hospital, advancing into a management position, overseeing the outpatient rehabilitation, sports medicine, cardiac pulmonary rehabilitation, and pain programs. Mrs. Taylor has had experience in rehabilitation of a variety of conditions, including cardiovascular attack (CVA), brain injury, brain tumor, spinal cord injury, Muscular Dystrophy, Amyotrophic Lateral Sclerosis (ALS), pain, orthopedics, and Sports Medicine. She is certified for adults Neuro-Developmental Treatment (NDT) and has had training in the treatment of Parkinson’s disease. Mrs. Taylor is also Augmented Soft Tissue Mobilization (ASTYM) certified, kinesotape trained, and has obtained a CEEAA (Certificate as Exercise Expert in Aging Adults) from the American Physical Therapy Association.
Mrs. Taylor graduated from University of Texas Health science Center at Dallas in 1976. She has continued her education with varies continuing education classes in both neurorehabilitation and orthopedics. She has also attended many management training classes. Mrs. Taylor also coauthored a book, Physical Therapy in Craniomandibular Disorder, with Dr. Tore Hanssen and Catherine Minor, PT. This has given her the opportunity to teach both in the States and abroad, given that the book has been published in five languages.
Janice Huey, OTR/L
Mrs. Huey is a state licensed and certified Occupational Therapist. She has extensive experience and training in the assessment and treatment of a variety of neurological disorders and associated deficits, pain management, and community reintegration. Mrs. Huey has worked in neurological rehabilitation inpatient and outpatient at Banner Good Samaritan treating patients with instrumental activities of daily living, work adaptations, on site job visits, and job related activities. She has also provided such treatment service at John C. Lincoln Hospital Neuro Rehab and in home health.
Mrs. Huey received her undergraduate degree in Occupational Therapy at the University of Southern California and completed fieldwork at BNI in Phoenix and mental health fieldwork in Los Gatos, California. She has advanced training in Cognitive Rehabilitation and Certification in Mental Health Counseling. She is also currently working in Out Patient Clinics.
Jamie Suppanz, M.S., CCC-SLP
Jamie Suppanz is a certified Speech-Language Pathologist. Prior to coming on board with Arizona Neurorehab, Mrs. Suppanz worked for over seven years serving adult individuals with various cognitive-communication and swallowing disorders in inpatient rehabilitation settings.
Mrs. Suppanz received her undergraduate degree in Audiology and Speech Sciences from Purdue University in West Lafayette, Indiana, where she graduated with honors. She obtained her Master’s Degree in Communication Disorders from Arizona State University, and completed her internship at Desert Samaritan Hospital and Desert Samaritan Care Center, both in Mesa, Arizona. Her Clinical Fellowship Year was at LifeCare Center of Scottsdale, Scottsdale, Arizona,
Mrs. Suppanz is state certified and has been a member of the American Speech and Hearing Association since 2003.
